This Sources Sought/Market Survey Notice is being conducted by Aleda E. Lutz VA Medical Center, 1500 Weiss St., Saginaw, MI 48602 to identify all sources that are capable of providing Post Discharge Patient Data Collection Services for the period of July 1, 2024 June 30, 2025 with four option years exercisable solely at the discretion of the Government in strict accordance with all schedules, specifications, terms, conditions and provisions of this contract.
DESCRIPTION / SPECIFICATIONS / WORK STATEMENT

Contractor shall supply all labor, materials, supervision, and software in order to provide discharge outcome data related to patient’s rehabilitation hospitalization for Commission on Accreditation of Rehabilitation Facilities (CARF) Accreditation and provide quarterly reports posted on-line to a secure site that can be retrieved by select VA employees who have been granted access via secure passwords. The period of performance shall be for one year with the Government’s option to exercise four (4) one (1) year option periods. This service is provided for the ALEDA E. LUTZ VAMC located in SAGINAW, MICHIGAN.

CONTRACTOR RESPONSIBILITIES

1. Contractor shall provide the following standard service to ALEDA E. LUTZ VAMC:
Data collection;
Data entry;
Referral to facility for identified patient concerns, problems or questions;
Standard quarterly report and list of unobtainable records;
Standard annual report;
Data collection on ten additional close-ended questions with an online report to the facility published on a monthly basis.

2. Data shall be collected by telephone interview approximately ninety (90) days following patient discharge with a minimum discharge to follow-up interval of eighty (80) days.

3. Contractor shall collect all information necessary to complete the follow-up coding sheet.

4. Contractor shall make up to five (5) attempts to contact each patient/caregiver for data collection. The data source may be either the patient, a family member, paid attendant or other individual who is knowledgeable regarding the patient’s functional status. Contractor uses the source for data collection that is provided by the VA.

5. Contractor shall provide an informational service only for the purpose as determined by the VA facility.

6. Contractor shall not be responsible for the medical interpretation of data collected, or the interpretation, use or consequences of use of this data by the VA facility.

7. Contractor agrees that all information disclosed to them is confidential in nature and that it shall not disclose such information to any third party without the prior written consent of the VA facility Contracting Officer’s Technical Representative (COTR) whose consent may be withheld, at the sole discretion of the VA facility. Data shall be released by the contractor to the participating VA facility through a designated representative of the participating facility.

8. Contractor’s service representative (telephone interviewer) is authorized by the VA facility to conduct said telephone interviews on behalf of the VA facility, and shall identify themselves as such to the patient/caregiver.

QUALIFICATIONS
1. Personnel performing contract services shall be fully qualified and competent, Functional Independence Measure (FIM credentialed/trained Nurse. “Fully qualified” Is defined as trained and experienced to successfully provide the contracted services as required herein.

The Contractor shall meet the following minimum requirements for performance:

Functional Independence Measure (FIM) instrument credentialed facility.
Have successfully processed FIM data/assessments according the Uniform Data System standards.
Sufficient knowledge and experience to interface effectively with VA computer systems.
Credentialed personnel for scoring of the FIM instrument.
The Contractor shall provide written verification of the competency of their personnel and a list of credentials of approved personnel actively providing services. Documented proof shall be submitted to the Contracting Officer with the Vendor’s quotation.
Contractor shall be a licensed subscriber of the Uniform Data System for Medical Rehabilitation, a division of UB Foundation Activities, Inc.
