SUBJECT: AFRH Operational Assessment (Mock Survey)
Request for Information (RFI)
Greetings,
The purpose of this Request for Information (RFI) is to find capable vendors to provide an Operational Assessment (Mock Survey) to be conducted in accordance with current Joint Commission standars, for Ambulatory Care, Home Care, Nursing Care, Life Safety/Environment of Care and Assisted Living Services to the Armed Forces Retirement Home. This Mock Survey will also need to provide an Overall Facility Review of procedures, practices, and an assessment of, as it relates to, clinical and safety processes. Please see the attached draft Performance Work Statement (PWS). Respondents to this RFI shall be able to perform all the duties/requirements listed in the attached portions of the Performance Work Statement (PWS).
This is merely a RFI for market research purposes, and not a solicitation.
INSTRUCTIONS TO RESPONDENTS
Respondents shall provide their Capability Statement fully illustrating their ability to meet the PWS requirements to [email protected] with the subject line: RFI 20342324N00005, Attn: SDaggett/GLJohnson – Mock Surveys no later than Thursday May 2, 2024 by 12:00 p.m. EST. Any other submissions will not be considered.
Interested vendors are asked to provide the following in their responses to this RFI:
1. The name, location, DUNS/UEI, and Point-of-Contact information (name, phone number, and email) for the respondent’s vendor.
2. A brief Capabilities Statement, which includes a description of the respondent vendor’s standard line of business and relative experience.
3. Include respondent vendor’s business size: Small or Other Than Small (OTS)/Socioeconomic Category.
Any questions about this RFI will not be entertained, as a vendor pool is all that is being sought at this time. Questions and Answers will be fielded during the RFP period in the future. Proposals will not be accepted at this time.
Only complete responses, submitted to the e-mail address above, by the established due date, will be considered. Any packages submitted after the established closing date and time will not be guaranteed consideration.
