This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This solicitation is issued as an RFQ.  The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06 (eff. 7/12/2021).

This solicitation is set-aside for Service-Disabled Veteran Owned Small Business Concerns only.

The associated North American Industrial Classification System (NAICS) code for this procurement is 334517, Irradiation Apparatus Manufacturing, with a small business size standard of 1,000 employees. The FSC/PSC is 6525, Imaging Equipment and Supplies: Medical, Dental, Veterinary.

The New Mexico VA Health Care System is seeking to purchase a BRAND NAME – Philips Healthcare; NCTD076 – CT Interventional Cart.

Interested Service-Disabled Veteran Owned Small Business concerns must have a signed Letter of Authorization from Philips Healthcare, specifying that your company is an authorized distributor of their equipment.

All interested companies shall provide quotations for all items included in Attachment D, Price and Delivery schedule.

Delivery shall be provided no later than 60 days after receipt of order (ARO); FOB Destination.

All quoters shall submit their quotes on company letterhead to include the delivery schedule with their pricing.

All quotes shall be sent to Darryl Handberg at darryl.handberg@va.gov.

Award will be based upon lowest price and adherence to statement of work in accordance with the Simplified Acquisition Procedures of FAR Part 13.

The award will be made to the response that is most advantageous to the Government. Responses should contain your best terms and conditions.

To facilitate the award process, all quotes must include a statement regarding the terms

and conditions herein as follows:

"The terms and conditions in the solicitation are acceptable to be included in

the award document without modification, deletion, or addition."

"The terms and conditions in the solicitation are acceptable to be included in

the award document with the exception, deletion, or addition of the following:"

Quoters shall list exception(s) and rationale for the exception(s), if any.

Submission of your response to this solicitation shall be received not later than 17 September 2021 @ 13:00 EST at darryl.handberg@va.gov. No other means of communication will be

accepted.

Late submissions shall be treated in accordance with the solicitation provision at FAR

52.212-1(f).
