Sources Sought Notice
2.Purpose: This Sources Sought/Market Survey Notice is being conducted by the Department of Veterans Affairs, Network Contracting Office (NCO) 16 in support of the G.V. (Sonny) Montgomery VA Medical Center located in Jackson, Mississippi to identify other sources that are capable of providing Patient Lifts.
3.Objective: To find interested vendors with the capability to provide brand name or equal equipment GH2 Upgrade to GH3 as listed below:
MANUFACTURER: Guldmann, Inc.
MODELS: Product # GH3+ 550lbs SWL+ Hanger Bar and conversion trolley
4. The North American Industry Classification (NAICS) code for this acquisition is 339113 Surgical Appliance and Surgical Manufacturing. Standard in number of employees is 750.
Contractor shall be an authorized vendor, authorized distributor, authorized by Guldmann, Inc., for proprietary equipment to meet all government requirements and objectives. All questions shall be answered in Section 5 for the government considered contractor with the capability to meet the government requirement.

5. Responses Required:
The G.V. Sonny VAMC requires all the following questions answered in this sources sought notice. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Jeneice McClinton, email@example.com by 12:00 pm (Central Time), March 23, 2021. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following:
5.1. Business Size and Socio-Economic Status:
(a) Indicate whether your business is large or small
(b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business
(c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act
(d) Indicate if your firm is a certified Hub zone firm
(e) Indicate if your firm is a woman-owned or operated business
(f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB)
(g) Include the DUNS number of your firm.
(h) State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov . If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive credit as a SDVOSB or VOSB status you must be registered in the VetBiz Registry.
Answers that are not provided shall be considered non-responsive to the Sources Sought/Market Survey Notice and Contractor shall not be considered as part of the market research.
5.2 Are you a Guldmann, Inc., authorized vendor or authorized distributor for providing equipment and delivery of medical equipment of patient lifts and capable to meet all government requirements and objectives? (Contractor shall attach an authorization letter or other documents from the Original Equipment Manufacturer (OEM), such that the OEM s software licensing, warranty and service provided and maintained by the OEM to be reviewed by the government to be considered as part of the Market Research and responsive to this Sources Sought notice).
5.3. Do you have an existing Blanket Purchase Agreement (BPA), General Services Administration Schedules (GSA) or Veteran Affairs (VA) Federal Supply Schedule (FSS) contract? If yes, please include the contract details and a brief summary of the products and services provided in your response.
5.4. Is your company capable of meeting all software licensing, warranty and service associated with the equipment and/or services such that the system shall be in accordance with the OEM terms and conditions?
Statement of Capability: Provide a short summary of your potential approach to this type of contract and meeting the specific requirements, delivery and your experience managing similar contracts with similar requirements for the G.V. Sonny VAMC.
6. Instructions and Response Guidelines:
Sources sought responses are due by March 23, 2021 at 12:00pm (Central Time); size is limited to 8.5 x 11 inches, 12-point font, with 1-inch margins in Microsoft Word format via email to firstname.lastname@example.org .
