SOW for Kavo OP 3D Pro
Purpose
The Clement J. Zablocki VAMC is requiring a new cone beam CT in the dental department. The cone bean CT should be able to take panoramic images of the mouth. This technology is important to plan for implant surgeries and treatment options for the mouth. The images should be able to be transferred to our dental pacs.
Hours of Operation
Normal hours of coverage are Monday through Friday, 7:00 AM to 4:30 PM Central Standard Time excluding national holidays. All services shall be performed during normal hours unless otherwise scheduled with the COTR or his/her designee. Contractor shall always be professional and shall take all necessary precautions to maintain a safe environment for the occupants. Contractor shall fully minimize noise possible while working in all buildings.
Note: Hardware/software update/upgrade installations, where applicable, will be scheduled and performed outside normal hours of coverage at no additional charge to the Government (unless it would be detrimental to equipment up-time; to be determined by the COTR).

Contractor will provide a warranty that is 1 year including parts and labor. VA shall have the option to buy an extended warranty of 4 years for parts/services. The equipment should be free from defects from materials and workmanship under normal use for a period of not less than 1 year. The warranty includes 24/7 access to telephone support. The warranty should start when the vendor and VA agree to a day of acceptance. Response time for telephone support should be within 4 hours of being contacted during our normal business hours. Response time for parts or service should be within 48 hours of being contacted.

Deliverables
The contractor shall deliver and install the product. After contacting COR (or COR designee) at the VA, a specific time will be scheduled for the delivery of the product and the installation within 60 days of award. All quantities are listed above that should be delivered.

The place of performance will be at Clement J Zablocki VA Medical Center, Milwaukee, WI.
Period of Performance
All hardware and software shall have a full warranty period of 12 months. VA shall be entitled to all software updates to fix bugs and safety issues if that version of software is being supported by Vendor.
