This is a Sources Sought Notice only and is issued in accordance with Federal Acquisition Regulation (FAR) 15.201(e) in order to conduct market research and gain knowledge of current market capabilities. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. OVERVIEW:

The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 2, and the Syracuse VA Medical Center are seeking potential qualified contractor(s) to provide all of the services related to operating and staffing a VA Patient Aligned Care Team (PACT) model capitated Community Based Outpatient Clinic (CBOC) in St. Lawrence County, New York.

Detail your firm s capability to deliver primary care and certain specialty services in a CBOC setting, ensuring you include responses to the Specific Response Items listed in the section below.

Prospective Contractors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM prior to award of a contract.
